The Presidential Health Summit and National Health Insurance: There can be no turning back in realising Universal Health Coverage in South Africa

The National Health Insurance (NHI) has been a topic of debate and discussion in South Africa for several years. However, at the recent 2023 Presidential Health Summit, Minister Joe Phaahla reiterated the government's commitment to the NHI, stating that there is no turning back on its implementation. The Congress of South African Trade Unions’ (COSATU) 1st Deputy President Michael Shingange also voiced strong support for the NHI, going further to reprimand those who still attempt to lobby against the strategic objectives of the new system. Without a doubt, South Africa has reached the point of no return in terms of its health reforms. The arc of history is firmly bent towards realising universal health coverage (UHC) through the national health insurance.

Universal health coverage models aim to provide all citizens with access to affordable and high-quality healthcare services, regardless of their income level. There are several types of universal health coverage models, including national health insurance, social health insurance, and national health service. Each model has its advantages and disadvantages, but national health insurance is often considered the most effective and efficient.

National health insurance is a single-payer system in which the government is responsible for financing and administering healthcare services. Under this system, all citizens pay into a central fund, which is used to cover the cost of healthcare services for everyone. In South Africa, the NHI will replace the current two-tiered healthcare system, which often leaves many citizens without access to essential healthcare services.

One of the key advantages of the NHI is that it will provide equal access to healthcare services for all citizens, regardless of their socio-economic status. This will help to reduce health inequalities and ensure that everyone has access to the same level of care. Under the current system, healthcare funding is largely based on the extractive and unsustainable fee-for-service model which encourages profit seeking behaviour, leading to over-utilisation. Under the NHI, healthcare funding will be based on the actual healthcare needs of the population, which will help to ensure that resources are allocated more efficiently.

While the NHI is not a perfect system and there will be challenges in its implementation, it is clear that it is the best way forward for South Africa's healthcare system. The government's commitment to the NHI and COSATU's support for it demonstrate that it is a non-negotiable aspect of the country's healthcare system.
